Terms & Conditions

Please read these terms and conditions and a subscription form follows.

This Subscription Agreement is made between:

  1. Coastal Goddess Pty Ltd ACN 647 990 458 of 8 Mimosa Ave, Saratoga NSW 2251; and
  2. The person named in Item 1 of the Schedule.

Operative Provisions:

  1. Definitions

In this agreement, the following words have the following meanings:

(a)    Fees means the Joining Fee and the Monthly Fee.
(b)    Force Majeure Event means any occurrence or omission as a direct or indirect result of which the party relying on it is prevented from or delayed in performing any of its obligations under this agreement and which is beyond the reasonable control of that party and could not have been prevented or mitigated by reasonable diligence or precautionary measures, including forces of nature, natural disasters, acts of terrorism, riots, revolution, civil commotion, epidemic, industrial action and action or inaction by a government agency.
(c)    Joining Fee means the fee set out in Item 4 of the Schedule.
(d)   Monthly Fee means the fee set out in Item 5 of the Schedule.
(e)   Services means the services to be provided by us to you under this agreement as set out in Item 3 of the Schedule.
(f)    Term means 12 months from the date of this agreement as set out in Item 2 of the Schedule.
(g)    We, Us or Our means Coastal Goddess Pty Ltd ACN 647 990 458 and its successors and assigns.
(h)    You or Your means the person named in Item 1 of the Schedule.

  1.    Services

2.1 We agree to make the Services available to you during the Term in consideration for payment of the
Fees.
2.2 The Services may include consultations, health assessments, treatment planning, educational resources,
supplementation guidance, in-clinic treatments, and other longevity-related services as determined by us.
2.3 Specific medical treatments (if applicable) will be discussed during consultation and are subject to
separate clinical consent documentation.
2.4 You are responsible for booking appointments. All appointments are subject to availability.

2.5 A minimum of 24 hours’ notice is required for cancellations. Failure to attend or late cancellation may
result in forfeiture of that appointment and we may refuse further Services.
2.6 Services must be utilised within the Term. Unused Services are forfeited and are not refundable or
transferable.
2.7 We reserve the right to modify the structure or delivery format of the Services where reasonably
necessary to maintain clinical standards or business operations.
2.8 After expiry of the initial 12 month Term, the agreement will continue on a month-to-month basis unless
terminated with 30 days’ written notice.

  1.   Fees

3.1 You must pay: (a) The Joining Fee on commencement; and
(b) The Monthly Fee as set out in the Schedule.
3.2 Monthly Fees are payable via direct debit or automatic payment method nominated by you.
3.3 Fees are inclusive of GST.
3.4 We may vary the Monthly Fee after the initial 12 month Term by providing 14 days’ written notice.
3.5 If payment is not received within 14 days of the due date, we may:
(a) Suspend Services;
(b) Require prepayment for future Services;
(c) Charge interest at 10% per annum calculated daily;
(d) Terminate the agreement.

  1.   Your obligations

4.1 You must: (a) Provide accurate and complete health information;
(b) Inform us of any changes to your health, medications or medical conditions;
(c) Follow reasonable clinical recommendations;
(d) Seek independent medical advice where recommended;
(e) Notify us promptly of any adverse reaction.
4.2 You acknowledge that optimal longevity outcomes require personal compliance and lifestyle
participation.

  1. Medical Disclaimer

5.1 The Longevity Project is designed to support health optimisation and wellbeing.
5.2 We do not guarantee specific outcomes, results, or improvements in biomarkers, symptoms, weight,
energy, or lifespan.
5.3 Nothing in this agreement replaces care from your general practitioner or specialist.
5.4 Certain services may require separate informed consent documentation prior to treatment.

  1. Warranties, Liability and Indemnity

6.1 We warrant that Services will be delivered with reasonable care and skill as required under Australian
law.
6.2 To the extent permitted by law, we exclude all warranties not expressly stated.
6.3 Our liability for breach of any consumer guarantee under the Australian Consumer Law is limited to:
(a) Re-supplying the Services;
(b) Paying the cost of re-supply; or
(c) Refunding the Fees paid.
6.4 We are not liable for: (a) Indirect or consequential loss;
(b) Loss of income, profits, business opportunity or goodwill;
(c) Outcomes arising from failure to follow medical or lifestyle advice;
(d) Adverse reactions where full medical history was not disclosed.
6.5 Except in cases of death or personal injury caused by negligence, our total liability will not exceed the
Fees paid by you in the preceding 12 months.
6.6 This clause survives termination.

  1. Termination

7.1 This agreement continues for the Term and thereafter until terminated with 30 days’ written notice.
7.2 You may not terminate during the initial 12 month Term unless:
(a) Required by law; or
(b) We agree in writing due to extenuating circumstances.

7.3 We may terminate immediately if:
(a) You breach the agreement and fail to remedy within 7 days;
(b) You provide false or misleading medical information;
(c) We determine continued participation is clinically inappropriate.
7.4 No refunds are payable for unused Services unless required by law.

  1. Privacy

8.1 You consent to us collecting, storing and using your personal and health information for the purpose of
delivering Services.
8.2 Health information will be handled in accordance with Australian Privacy laws.
8.3 De-identified data may be used for internal quality improvement purposes.

  1. General

9.1 We may amend these Terms after the initial 12 month Term with 14 days’ notice.
9.2 This agreement constitutes the entire agreement between the parties.
9.3 If any provision is unenforceable, it will be severed without affecting the remainder.
9.4 Notices must be in writing.
9.5 This agreement is governed by the laws of New South Wales, Australia.

  • Specific inclusions vary by membership tier and are outlined at the time of purchase.
